The short version

Polycystic ovary syndrome is common in Nigeria and frequently undertreated. Its central metabolic feature, insulin resistance, is exactly the thing GLP-1 medicines act on, which is why interest in them has grown so quickly among Nigerian women.

What the evidence supports

International PCOS guidance published in recent years includes GLP-1 receptor agonists as an option for women with PCOS and obesity where lifestyle change and metformin have not achieved enough. Reviews of the evidence report improvements across three areas: body weight, insulin sensitivity and menstrual regularity.

Studies have also reported restoration of ovulation in a substantial share of treated women, and comparative data suggest GLP-1 medicines improve insulin resistance markers more than metformin alone.

What that means practically

The fertility point matters. Women who have not conceived for years sometimes ovulate again within months of starting treatment. If pregnancy is not your plan, arrange reliable contraception before you start. If it is your plan, that changes the medication decision entirely.

Contraception and these medicines

Tirzepatide slows gastric emptying enough to reduce absorption of oral hormonal contraceptives, and the effect is largest after the first dose. The manufacturer advises switching to a non-oral method, or adding a barrier method, for four weeks after starting and for four weeks after each dose increase.

Non-oral methods, including implants, injections and hormonal coils, are not affected in the same way.

Pregnancy

Neither semaglutide nor tirzepatide is for use in pregnancy. Because semaglutide stays in the body for a long time, the manufacturer advises stopping it at least two months before a planned pregnancy. For tirzepatide, ask your doctor how long to leave between stopping and trying to conceive.

If you conceive while taking either, tell your doctor promptly rather than waiting for your next appointment.

What to sort out before starting

  1. Get a proper diagnosis. Irregular cycles have several causes, and thyroid problems in particular are commonly mistaken for PCOS.
  2. Check the contraindications with your doctor, including any personal or family history of medullary thyroid cancer or MEN 2.
  3. Decide the contraception question before your first injection, not after.
  4. Plan for protein and resistance training. Insulin sensitivity improves with muscle, and muscle is what you lose if you do not plan against it.
  5. Budget realistically. Stopping after two months tends to undo the metabolic gains along with the weight.

Our guide on expected weight loss sets out what the trials found for weight specifically, which is the lever behind most PCOS improvement.
